Helping You Through Radiation
Because radiation therapy is such a precise treatment, patients must remain very still for up to an hour during simulation and treatment. Most of our patients do not require sedation for radiation therapy. If you are able to complete an MRI awake, it is likely you can also complete radiation awake.
For our youngest patients, holding still for 30-60 minutes can be too difficult, so our on-site anesthesia team will provide sedation to ensure your treatment is completed safely and accurately.
If you need sedation, a nurse will place an IV or access your central line when you arrive and then bring you into this room. If you do not have a port or PICC line on the day of simulation, you may receive anesthesia through a mask. We will coordinate with your primary hospital team to have a port or PICC line placed before treatment starts.
We will keep you next to your loved ones as long as possible, and they will be called back to the bedside as soon as you start to wake up.

Helping You Through Radiation
Our specialty-trained pediatric nurses and nurse practitioners are available daily while you are on treatment to discuss how things are going, address potential side effects, give medications, and respond to other questions you or your family may have.
The majority of our pediatric patients complete radiation without sedation. You must remain entirely still during treatment, similar to when you have an MRI. If you can complete an MRI without sedation, you can likely complete radiation awake as well. If you do require sedation to complete radiation treatment, our nurses are trained in pediatric anesthesia and work with our pediatric anesthesiologists to provide this option.

Pediatric Nurses
Our radiation program treats nearly 200 pediatric patients annually, referred from medical institutions worldwide. Our patients come to us with a wide range of diagnoses, from brain tumors to solid tissue tumors and sarcomas, leukemia, lymphomas, and even non-malignant disorders, just to name a few. Our pediatric radiation program benefits from a strong network of pediatric physicians, staff and programs, including our specially trained clinical staff, child life specialists, volunteer support, and a generous charitable fund.
Depending on the treatment plan, we will help children through treatment with programs such as audio and video in treatment rooms and Hole in the Wall Gang in-hospital “camp” days.

Pediatric Proton Therapy
Johns Hopkins Medicine and Children’s National have collaborated to bring dedicated pediatric radiation oncology/pediatric proton therapy to our region. Our team offers specialized clinical expertise as well as dedicated pediatric staff, daily assigned anesthesia providers for our smallest patients, and a child-friendly environment.
Our team of radiation oncologists, nurses, and anesthesiologists specialize in pediatric care. We also offer our patients the assistance of a child life specialist and a pediatric social worker.
Together, we will work to assess your needs and provide developmentally appropriate preparation and support for treatment.
We know the challenges many of our patients face and are prepared to assist you through the process from referral to bell ceremony.

Preparation
Preparing for radiation treatment requires two pre-treatment appointments: a consultation and a simulation.
During the consultation, you and your family will speak with the radiation oncologist to discuss your specific needs related to your diagnosis.
Simulation is a planning appointment. You’ll learn more about simulation in the CT room of this resource.

Proton Therapy
Proton therapy begins with a huge particle accelerator, known as a synchrotron, that spins protons at ultra-fast speeds before making their journey along a beam line to our treatment rooms.

Having 360° treatment gantries means that we can be very precise in our treatment, even in the hardest to reach areas.
Using pencil beam scanning technology, proton therapy is the most precise radiation treatment currently available, potentially sparing surrounding tissues and vital organs from excess radiation and the related side effects.

Sedation
If needed on simulation or treatment days, this is where a nurse would access your port or place an IV. You will need a medication line, like an IV or port, if you are being sedated or need a contrast. If you need an IV or your port needs to be accessed, we can use tools like numbing cream to help minimize pain or discomfort.

Simulation (MRI)
Understanding the make-up of a tumor or disease process and the surrounding structures is critical to targeting them with radiation therapy.
Magnetic resonance imaging (MRI) can produce detailed images of almost every internal structure in the human body–including organs, bones, muscles, and blood vessels–using a large magnet and radio waves.
MR images are used to precisely delineate the treatment field and adjacent normal tissues. Image-based treatment planning allows for better targeting cancer, especially in moving tumors such as those in the heart, lungs, and abdomen, which move when breathing.
The MRI during simulation is taken for treatment planning, not for diagnostic purposes.

Targeting Your Tumor Before Treatment
Understanding the make-up of a tumor or disease process and the surrounding structures is critical to targeting them with proton therapy.
Computerized tomography (CT) imaging uses a combination of X-rays and computer technology to produce 3D images that are more detailed than standard X-rays. These images help the treatment plan team visualize tissue depth and serve as the basis to create a radiation treatment plan.
The CT during simulation is taken for treatment planning, not for diagnostic purposes.

Who is the Peds Radiation Team?
Proton therapy depends on the collaboration of a multi-disciplinary team. At Johns Hopkins, you will be cared for by team members in the clinic and many others behind-the-scenes. You’ll become familiar with your radiation oncologist, nurse practitioner, nurses, nurse navigator, child life specialist, social worker, radiation therapists, and administrative team.
Although you might not meet them, your treatment will also benefit from the expertise of our medical physicists, dosimetrists, engineers, and researchers.
